Policy

MEDICALLY NECESSARY

Options and accessories for wheelchairs are considered medically necessary and, therefore, covered if an individual has a wheelchair that meets Company coverage criteria and the option/accessory itself is medically necessary. In addition, medical necessity criteria apply for certain accessories, as follows:

ARM OF CHAIR
Detachable, adjustable arm height options (E0973, K0017, K0018, K0020) are considered medically necessary and, therefore, covered when an individual requires an arm height that is different than the available height using a nonadjustable arm, and the individual spends at least 2 hours per day in the wheelchair.

An arm trough (E2209) is considered medically necessary and, therefore, covered when an individual has quadriplegia, hemiplegia, or uncontrolled arm movements.

FOOT REST/LEG REST
Elevating leg rests (E0990, K0046, K0047, K0053, K0195) are considered medically necessary and, therefore, covered when any of the following criteria are met:
  • The individual has a musculoskeletal condition or a cast or brace that prevents 90-degree flexion at the knee.
  • The individual has significant edema of the lower extremities that requires an elevating leg rest.
  • The individual meets the criteria for, and has a reclining back on, the wheelchair.
NONSTANDARD SEAT-FRAME DIMENSIONS
A nonstandard seat width and/or depth (E2201 through E2204) for a manual wheelchair is considered medically necessary and, therefore, covered if the individual's dimensions justify the need.

WHEELS/TIRES FOR MANUAL WHEELCHAIRS
Gear-reduction drive wheels for a manual wheelchair (E2227) are considered medically necessary and, therefore, covered when all of the following criteria are met:
  • The individual has been self-propelling in a manual wheelchair for at least 1 year.
  • The individual has had a specialty evaluation that was performed by a licensed/certified medical professional, such as a physical therapist (PT) or occupational therapist (OT), or by a professional provider who has specific training and experience in rehabilitation wheelchair evaluations.
  • The wheelchair is provided by a supplier that employs a Rehabilitation Engineering and Assistive Technology Society of North America (RESNA)-certified Assistive Technology Supplier (ATS) or Assistive Technology Professional (ATP) who specializes in wheelchairs and who has direct, in-person involvement in the wheelchair selection for the individual.
BATTERIES/CHARGERS
Up to two sealed batteries (E2359, E2361, E2363, E2365, E2371, K0733) at any one time are considered medically necessary and, therefore, covered if required for a power wheelchair.

POWER TILT AND/OR RECLINE SEATING SYSTEMS (E1002 through E1010, E1012)
A power-seating system (tilt only, recline only, or combination tilt and recline) with or without power elevating leg rests is considered medically necessary and, therefore, covered if an individual meets ALL of the following medical necessity criteria for a power wheelchair as described in the policy that addresses power-mobility devices and the following criteria:
  • A specialty evaluation, including the documentation of the individual’s seating and positioning needs, was performed by a licensed/certified medical professional, such as a PT or OT, or a professional provider who has specific training and experience in rehabilitation wheelchair evaluations.
  • The seating system must be provided by a supplier that employs a RESNA-certified ATP who specializes in rehabilitation wheelchairs and who has direct, in-person involvement in the selection of the seating system for the individual.
  • The individual meets one of the following criteria:
    • The individual is at high risk for development of a pressure ulcer and is unable to perform a functional weight shift.
    • The individual utilizes intermittent catheterization for bladder management and is unable to independently transfer from the wheelchair to the bed.
    • The individual needs the power seating system to manage increased tone or spasticity.
POWER WHEELCHAIR DRIVE-CONTROL SYSTEMS
An attendant control (E2331), instead of a standard drive-control system, is considered medically necessary in place of an individual-operated drive-control system, when ALL of the following criteria are met:
  • The individual meets all of the medically necessity criteria for a wheelchair
  • The individual is unable to operate a manual or power wheelchair
  • The individual has a caregiver who is unable to operate a manual wheelchair but is able to operate a power wheelchair.
OTHER POWER WHEELCHAIR ACCESSORIES
An electronic interface (E2351) that operates a speech-generating device using a power wheelchair control interface is medically necessary when an individual has a medically necessary speech-generating device.

MISCELLANEOUS ACCESSORIES
An anti-rollback device (E0974) is considered medically necessary and, therefore, covered when an individual self-propels and needs the device because of ramps.

A safety belt/pelvic strap (E0978) is considered medically necessary and, therefore, covered when an individual has weak upper body muscles, upper body instability, or muscle spasticity that require the use of this item for proper positioning.

One example (not all-inclusive) of a medically necessary use for swingaway, retractable, or removable hardware (E1028) would be to move the component out of the way so that an individual could perform a transfer to a chair or bed.

A manual, fully reclining back (E1226) is considered medically necessary and, therefore, covered when one or both of the following conditions is present:
  • The individual is at high risk for development of a pressure ulcer and is unable to perform a functional weight shift.
  • The individual utilizes intermittent catheterization for bladder management and is unable to independently transfer from the wheelchair to the bed.
For information concerning a power assist system for a manual wheelchair, refer to the Power Mobility Devices medical policy: Power Wheelchairs (PWCs), Power-Operated Vehicles (POVs), and Push-Rim Activated Power-Assist Devices.

NOT MEDICALLY NECESSARY

If the criteria for the specific item(s) described above are not met, the item(s) are considered not medically necessary and, therefore, not covered.

An option/accessory that is beneficial primarily in allowing an individual to perform leisure or recreational activities is considered not medically necessary and, therefore, not covered.

A nonsealed battery (E2358, E2360, E2362, E2364, E2372) is considered not medically necessary and, therefore, not covered.

A single-mode battery charger (E2366) is appropriate for charging a sealed lead acid battery. However, if a dual-mode battery charger (E2367) is provided as a replacement, it is considered not medically necessary and, therefore, not covered.

If an attendant control (E2331) is provided in addition to a patient-operated drive-control system, it is considered not medically necessary and, therefore, not covered.

REQUIRED DOCUMENTATION

The Company may conduct reviews and audits of services to our members regardless of the participation status of the provider. Medical record documentation must be maintained on file to reflect the medical necessity of the care and services provided. These medical records may include but are not limited to: records from the professional provider’s office, hospital, nursing home, home health agencies, therapies, and test reports.

STANDARD WRITTEN ORDER REQUIREMENTS
Before submitting a claim to the Company, the supplier must have on file a timely, appropriate, and complete standard written order for each item billed that is signed and dated by the professional provider who is treating the member. Requesting a provider to sign a retrospective standard written order at the time of an audit or after an audit for submission as an original standard written order, reorder, or updated order will not satisfy the requirement to maintain a timely professional provider order on file.

PROOF OF DELIVERY REQUIREMENTS
Medical record documentation must include a contemporaneously prepared delivery confirmation or member’s receipt of supplies and equipment. The medical record documentation must include a copy of delivery confirmation if delivered by a commercial carrier and a signed copy of delivery confirmation by member/caregiver if delivered by the DME supplier/provider. All documentation is to be prepared contemporaneous with delivery and be available to the Company upon request.

CONSUMABLE SUPPLIES (WHEN APPLICABLE)
The DME supplier must monitor the quantity of accessories and supplies an individual is actually using. Contacting the individual regarding replenishment of supplies should not be done earlier than approximately 7 days prior to the delivery/shipping date. Dated documentation of this contact with the individual is required in the individual’s medical record. Delivery of the supplies should not be done earlier than approximately 5 days before the individual would exhaust their on-hand supply.

For specified DME items, documentation of a face-to-face encounter between the treating professional provider and the individual meeting the above requirements, including an assessment of the individual’s clinical condition supporting the need for the prescribed DME item(s), must be provided to and kept on file by the DME supplier.

If required documentation is not available on file to support a claim at the time of an audit or record request, the DME supplier may be required to reimburse the Company for overpayments.

REPAIR AND REPLACEMENT
Requests for a different type of power mobility device (PMD) due to a change in medical and/or functional status such that the individual can no longer operate their present PMD are considered new requests, not requests for replacement. These requests are evaluated against the medical necessity criteria for the type of PMD that is requested.

For more information on criteria for the repair and replacement of PMDs, refer to the policy addressing the repair and replacement of DME.

Description

Wheelchair options and accessories are types of durable medical equipment (DME) that are used with mobility-assistive equipment (MAE) such as manual wheelchairs, power wheelchairs, and power-operated vehicles (POVs). Wheelchair options and accessories may be essential for the functionality of the MAE; they may also be medically necessary for the user's safety and/or ability to perform mobility-related activities of daily living (MRADLs) in the home. MRADLs such as toileting, feeding, dressing, grooming, and bathing usually take place in specific locations in the home. Mobility limitations may impact an individual's ability to participate in MRADLs in their customary locations within the home and/or to perform them in a timely and safe manner. The use of MAE, including options and accessories, may be appropriate to facilitate the performance of MRADLs in the home.

Wheelchair options and accessories may include, but are not limited to, armrests, footrests, wheels, tires, and drive-control systems. Some wheelchair options and accessories are included (ie, standard features) with MAE, while others are prescribed on an individual basis according to medical necessity criteria. In either case, a single option/accessory may apply, or a range of options/accessories may be necessary.
