THE EASTERN COOPERATIVE ONCOLOGY GROUP PERFORMANCE STATUS

The Eastern Cooperative Oncology Group (ECOG), established in 1955, was one of the first groups to coordinate multicenter cancer clinical trials. The National Cancer Institute (NCI) is the primary funding source, and ECOG has evolved from a small consortium of institutions in the eastern United States to one of the largest clinical cancer research organizations in the country. As part of their work in the treatment of cancer, ECOG has developed the ECOG Performance Status (PS), originally published in 1982 in the American Journal of Clinical Oncology. The use of the scales and the criteria in the ECOG PS allows clinicians and researchers to determine an individual’s disease progression in terms of how the activities of daily living (ADL) are affected.

ECOG Performance Status

  • Newly diagnosed unresectable regional nodal disease
  • Individuals unfit for surgery
  • Metastatic disease at initial presentation (M1)
  • Recurrent or persistent disease with or without distant metastases
US FOOD AND DRUG ADMINISTRATION STATUS

The US Food and Drug Administration (FDA) approved penpulimab-kcqx on April 23, 2025, in combination with either cisplatin or carboplatin and gemcitabine for the first-line treatment of adults with recurrent or metastatic nonkeratinizing nasopharyngeal carcinoma (NPC). At the same time, penpulimab-kcqx was approved as a single agent for the treatment of adults with metastatic NPC with disease progression on or after platinum-based chemotherapy and at least one other prior line of therapy.

PEDIATRIC USE
The safety and effectiveness of penpulimab-kcqx in pediatric individuals have not been established.

Description

NASOPHARYNGEAL CARCINOMA

Nasopharyngeal carcinoma (NPC) is a type of head and neck cancer that starts in the upper part of the throat (nasopharynx) behind the nose. The nasopharynx is the beginning part of the passageway for air traveling from the nose to the lungs. There are multiple types of NPC: keratinizing squamous cell carcinoma, nonkeratinizing differentiated carcinoma, nonkeratinizing undifferentiated carcinoma, and basaloid squamous cell carcinoma. NPC is a rare cancer in most parts of the world, including the United States, with fewer than one case for every 100,000 individuals. In parts of South Asia, North Africa, the Arctic, and the Middle East, however, there may be 15 to 30 cases per 100,000 individuals. The exact causes of NPC are not yet known, but there seem to be some risk factors that may play a role in the development of the cancer including diet, smoking, heavy alcohol use, genetics, and past infection with either the Epstein-Barr virus (EBV) or certain types of the human papillomavirus (HPV). Treatment usually includes pharmacologic therapy with or without radiation therapy. Surgery is not usually an initial treatment, but may occur later in the course of therapy.

PENPULIMAB-KCQX

Penpulimab-kcqx is a programmed death-1 (PD-1) receptor blocking IgG1 antibody. When the PD-1 receptor is bound by programmed death-ligand 1 or 2 (PD-L1, PD-L2) on the T cells, the T-cell immune surveillance of tumors will be inhibited, allowing tumors to grow. By binding to the PD-1 site, penpulimab-kcqx prevents the T cell from interacting with PD-L1 or PD-L2, and allows the T cell to continue its antitumor immune response.

CLINICAL TRIALS

FIRST LINE WITH PLATINUM AND GEMCITABINE
The efficacy and safety of penpulimab-kcqx, in combination with platinum and gemcitabine, as first-line therapy for recurrent or metastatic NPC was evaluated in a multicenter, randomized, double-blind, phase III clinical trial (NCT04974398). Participants were randomly assigned 1:1 to receive either systemic penpulimab-kcqx or placebo in combination with a platinum agent (cisplatin or carboplatin) and gemcitabine every 3 weeks for up to six cycles, followed by maintenance therapy with either penpulimab-kcqx or placebo. If any individual in the placebo cohort was confirmed to have progressive disease, they were allowed to change to therapy with penpulimab-kcqx. The primary endpoint was progression-free survival (PFS). Key secondary endpoints were overall survival (OS), objective response rate (ORR), and duration of response (DOR).

A total of 291 individuals were randomly assigned, with 144 receiving penpulimab-kcqx and 147 receiving placebo. At a median follow-up of 19.1 months, the median PFS for the penpulimab-kcqx cohort was 9.6 months versus 7.0 months for the placebo cohort (hazard ratio [HR], 0.45; 95% confidence interval [CI], 0.33–0.62; P<0.0001). OS was not mature. Confirmed ORR was 68.1% for the penpulimab-kcqx cohort and 63.9% for the placebo cohort. DOR was 9.8 months for the penpulimab-kcqx cohort and 5.7% for the placebo cohort (HR, 0.4, 95% CI, 0.27–0.59). Treatment-related adverse events (TRAE) of grade 3 or higher were experienced by 89.0% of individuals in the penpulimab-kcqx cohort and 85.9% of individuals in the placebo cohort.

THIRD LINE AS A SINGLE AGENT
The efficacy and safety of penpulimab-kcqx, as a single agent, for the treatment of individuals with metastatic non-keratinizing NPC with disease progression on or after platinum-based chemotherapy and at least one other prior line of therapy were evaluated in a multicenter, single-arm, open-label, phase II clinical trial (NCT03866967). Participants received penpulimab-kcqx every 2 weeks until disease progression or intolerable toxicities occurred. The primary endpoint was ORR. Key secondary endpoints were PFS, OS, and DOR.

A total of 125 individuals were enrolled. The ORR was 28% with one complete response and 27 partial responses. The median PFS was 3.6 months. The median OS was 22.8 months. The median DOR was not matured. TRAE were experienced by 71.5% of enrolled individuals.

OFF-LABEL INDICATIONS

There may be additional indications contained in the Policy section of this document due to evaluation of criteria highlighted in the Company's off-label policy, and/or review of clinical guidelines issued by leading professional organizations and government entities.
