US FOOD AND DRUG ADMINISTRATION (FDA) STATUS

Epcoritamab-bysp (Epkinly) was approved by the FDA on May 19, 2023, for the treatment of individuals with relapsed or refractory diffuse large B-cell lymphoma (DLBCL), not otherwise specified, including DLBCL arising from indolent lymphoma, and high-grade B-cell lymphoma after two or more lines of systemic therapy.  Supplemental approvals have since been issued by the FDA.

Epkinly is administered via subcutaneous injection, after step-up dosing, once every 28 day cycle, continuing until either disease progression or unacceptable toxicity.

INPATIENT ADMISSION

When the medical necessity criteria has been met, the inpatient admission for administration of epcoritamab-bysp (Epkinly), as recommended by the US Food and Drug Administration (FDA), is covered in accordance with the following timeframes*:

  • First Cycle - day 15 of therapy individuals are hospitalized for 24 hours after administration of 48-mg dosage due to the risk of cytokine-release syndrome (CRS) and immune effector cell–associated neurotoxicity syndrome (ICANS)

*Inpatient hospital stays that are longer than the preceding timeframes must be re-evaluated for medical necessity.

PEDIATRIC USE

The safety and effectiveness of epcoritamab-bysp (Epkinly)  in pediatric individuals have not been established.

Description

Diffuse large B-cell lymphoma (DLBCL)

Diffuse large B-cell lymphoma (DLBCL) is the most common subtype of non-Hodgkin lymphoma (NHL). DLBCL is one of the most frequent subtypes of aggressive lymphomas, which can become transformed lymphoma that mostly develops from other types of lymphoma. Individuals with DLBCL who are treated with first-line therapy can become refractory to therapy or relapse after initially responding to therapy. Individuals who become refractory to therapy, or relapse or progress after therapy, often have a poor prognosis.

FOLLICULAR LYMPHOMA

According to the American Cancer Society, about one of five lymphomas in the United States is a follicular lymphoma (FL). They are usually a slow-growing (indolent) lymphoma that occurs in adults aged 60 years and older.

Epcoritamab-bysp (Epkinly)

Epcoritamab-bysp is a bispecific T-cell–engaging antibody (Anti-CD20/CD3 antibody) that activates T cells by binding to the CD3 receptor, directing them to kill malignant CD20+ B cells. The label recommends two step-up doses of 0.16 mg and 0.8 mg during the first treatment cycle, followed by 48-mg full doses.

PEER-REVIEWED LITERATURE

Summary

DLBCL and High-grade B-cell Lymphoma

The approval for DLBCL was based on an phase I/II, open-label, multicohort, multicenter, single-arm multicenter study (First-in-Human [FIH] Trial in Patients With Relapsed, Progressive or Refractory B-Cell Lymphoma [EPCORE™ NHL-1]) that evaluated epcoritamab (subcutaneous) in individuals (n = 157) with relapsed or refractory DLBCL (including transformed indolent lymphomas, high-grade B-cell lymphomas, or primary mediastinal large B-cell lymphomas) after two or more lines of therapy (including an anti-CD20 mAb-based regimen, prior failure and ineligibility for high-dose therapy/autologous stem cell transplantation, and prior chimeric antigen receptor (CAR) T-cell therapy [≥30 days since last treatment]).  The median follow-up was 11 months. The primary efficacy endpoint was overall response rate (ORR) (assessed according to the Lugano 2014 criteria by the independent review committee was 61% [38% complete response (CR)]). The median progression-free survival (PFS) was 4 months (not reached in individuals who achieved CR) and the 6-month PFS rate was 44%. The median overall survival (OS) was not reached. Among the prespecified subgroups, the ORR was 55% (30% CR) for individuals with primary refractory disease and 54% (34% CR) for individuals who had received prior CAR T-cell therapy. Epcoritamab resulted in an ORR of 68% (45% CR) in individuals with transformed indolent lymphomas.  The ORR was higher in the subgroup of individuals who had not received prior CAR T-cell therapy (69%; 42% CR). Any-grade CRS was reported in 50% of individuals (grade ≥3; 3%). CRS occurred after the administration of first full dose of epcoritamab (Cycle 1, Day 15).

Follicular Lymphoma

EPKINLY as Monotherapy for FL

The approval for FL was based on a multicenter, open-label phase 1/2 study single-arm trial (EPCORE NHL-1) that included 127 participants with relapsed or refractory FL after at least two lines of systemic therapy. The primary endpoint was the overall response rate, which was met by 82% of participants, with 60% of participants achieving a complete response and 22% achieving a partial response. At a median follow-up of 14.8 months among patients who responded to treatment in the study, more than 50% of participants remained responsive to treatment (i.e., the median duration of response was not reached).

EPKINLY in Combination with Lenalidomide and Rituximab for FL

EPCORE FL-1 was a randomized, open-label trial that included 488 participants with R/R FL and a median of one prior line of systemic therapy. Participants were randomized (1:1) to receive Epkinly with R2 or R2 alone. Epkinly with R2 significantly improved efficacy outcomes: median progression-free survival was not reached versus 11.2 months (hazard ratio, 0.21), and the overall response rate was 89% versus 74%, with complete responses in 74% versus 43% of patients, respectively. Safety data from EPCORE FL-1 showed that Epkinly with R2 was associated with expected toxicities, including cytokine release syndrome (CRS; 24% any grade; 12% grade ≥3), neutropenia (66%), infections (29%), and rare events of immune effector cell–associated neurotoxicity syndrome (ICANS; <1%).
