The Company reserves the right to reimburse only those services that are furnished in the most appropriate and cost-effective setting that is appropriate to the member’s medical needs and condition.
This policy applies to professional and outpatient facility claims.
In certain circumstances, it may be necessary to identify that a service is distinct or separate from another service performed on the same date. In such cases, the Company requires submission of the most appropriate and specific modifier to accurately reflect the distinct nature of the service provided.
